[PATCHv2 1/2] NVMe: Reset failed controller

Polls on the controller fatal status bit and resets the controller per
the nvme spec if controller is failed. The controller reset is done only
if the device has completed initialization.
If the controller fails to start after attempting to reset it, the pci
driver will be removed since the controller is not usuable.
